Overview

GPNW currently supports General Practitioners and their staff to provide optimal care to patients with diabetes and contribute to the achievement of the best possible health outcomes for the community.


Sustained improvements in health outcomes for people with chronic diseases such as diabetes have been associated with a more systematic approach in general practice including intensive follow up, use of clinical management guidelines integrated with self-management support programs and more effective use of nurse case managers and non-physician care provider. Systematic care includes having a disease register, regular recall and review, protected time, a practice nurse, clear written guidelines and a system for auditing standards of care.

Diabetes Nurse Education to Residents of Low Care Aged Care facilities

General Practice North West (GPNW) has had a small amount of funding made available to enable one of our Diabetes Nurse Educator to visit Low Care Residential Aged Care facilities to provide assessment and education of residents with a diagnosis of Diabetes.

Information on access to this program has been forwarded to the Directors of Nursing/Care Co-ordinators of the Residential Aged Care facilities.

GPNW Diabetes Nurse Educator will primarily focus assessment and education of the resident but acknowledge the need to facilitate some staff education to ensure recommendations for resident management are supported and implemented.

GPNW Diabetes Nurse Educator will contribute to the residents facility care file.

Whilst a GP referral is preferred, it can also be completed by the Clinical Nurse Manager/Registered Nurse of the Low Care facility with GP knowledge.

More Allied Health Services Program (MAHS)

GPNW currently provides a Diabetes Clinical Services which includes two Diabetes Educators in order to :

  • Improve the health care of key groups within a rural community through the provision of efficient and effective allied health services which have been selected on the basis of the identified needs of the community.
  • Provide additional (in quantity or range) professional allied health services to rural communities.
  • To facilitate an integrated approach to health care provision by allied health care professionals and general practitioners working together to meet the care needs of patients, and by the Division linking the More Allied Health Services Program with other State/Australian Government funded health initiatives.

The Diabetes Educator provide clinics in all sectors of the North West including the West Coast, Circular Head and King Island. Clinics are predominantly held in the general practice setting, providing direct patient care and notes into Medical Director. Clinics are also held in community centres to ensure all communities are serviced.

The Diabetes Educator also provide education and upskilling to practice nurses on the care of patients with diabetes.

The service works collaboratively with State and private health providers to ensure an integrated care for patients. High quality resources and materials are also produced and distributed to general practices.

The Diabetes Educator attend professional development to ensure the highest level of professional care for patients under the service.

The service has established shared pathways for General Practitioners through collaboration with the State Services of both North West Diabetes and North West Dietetics services. Part of this partnership has been the establishment of referral templates for GPs to refer to either services.

Project Objectives

Future objectives of the project are to increase specialist services for patients with diabetes by accessing a clinical endocrinologist placement for the region and to provide education opportunities for GPs in areas relating to care of their patients.  Dr R McCallum will commence a clinical service in Queenstown from early September, providing a much needed service every six months to the West Coast.
