
Overview:
The Practice Health Atlas (PHA) is a decision support tool that aims to inspire general practice teams to reflect on their activities and to develop innovative business models for more effective health care services/outcomes. It is based in the combination of relevant, high quality and timely practice health data, as well as using such data to predict future health care needs and trends (intelligence).
This support tool has been developed specifically to extract de-identified patient information from a practice’s clinical software. GPNW then creates a personalised report for the practice, as well as a benchmarking and comparison report, allowing practices to compare their performance and against with their peers.
The report includes:
- Patient Demographics - Total active patients, age, gender, etc. A demographic comparison is also made with census data in their region. So if there is 10,000 people in the postcode area and the practice has 7,500 from same postcode they know they treat 75% of patients in that area.
- Chronic Disease Profiles - Looks at demographics for 6 chronic conditions (Asthma, COAD, Hypertension, Cardiovascular, Diabetes & Mental Health). Similar data from point 1 only it is specific to the chronic conditions. I.e.: 300 patients with Diabetes – average age 45, 200 male, 100 female etc.
- Business Clinical Modelling – this gathers information from the billing package around various EPC items (Health Assessments, Diabetes Assessments, Home Medication Reviews, Chronic Disease item numbers etc). From the billing package they identify which patients are eligible for Medicare item numbers and show actual vs potential item numbers claimed. They also look at the financial benefits of these item numbers. For e.g. If the practice has 750 patients who are eligible for Health Assessment and they have only completed 200 in the year – they work out the practice could do 550 more and the amount of money they could generate.
- Access to Services – this section is used to identify the types of allied health professionals the practice is likely to use based on chronic conditions and demographics and assists them to locate the services they require.

Basic instructions
Note:- While this tool can aggregate v7.5 and v8 data, the actual aggregated PHA (working Data folder) must be version 8.
- Extract the .mdb database and copy it to somewhere on your computer.
- Open the database, enable any security warnings, then follow steps 1 to 3 (in that order)
- Repeat for all the PHA's you want to aggregate
Creating the Aggregated PHA
- Make a new copy of a version 8 Working_Data folder
- Open the AtlasBuilder.mdb
- Go into the table list and DELETE the patient table
- Now you need to link back to the "patient" table that is located in the CombinedData.mdb file (this is where all the aggregated patient data is). To do this in Access 2007, click the External Data tab, Click the Access icon, browse to the CombinedData.mdb file and choose "Link to the data source by creating a linked table", then click OK
- Close the AtlasBuilder.mdb and continue creating your PHA as you normally do.
