The Rural Palliative Care Project aims to improve palliative care for patients in rural areas in North West Tasmania.
The main objectives of the project are:
- To increase access to quality palliative care services by fostering relationships and networking between general practice and other primary service providers of palliative care in network catchment area.
- Ongoing communication between specialist palliative care services.
- Strengthen link between dedicated palliative care teams, general practice and mainstream service providers.
- Identify need based education and skill enhancement opportunities for palliative care service providers and general practice.
- Establish a multidisciplinary team approach to facilitate collaborative care planning for palliative patients.
- Develop systems that will enable sustainability of palliative care services in rural areas.
